Carrying controlled medication into China: why the red channel, not the green, is the safe one

Opioids, ADHD stimulants and many sedatives are controlled drugs in China, whatever they are at home. Entry is possible for personal treatment, but only with a diagnosis letter, a prescription, original packaging, a declaration at the red channel and a reasonable quantity.

Travel·

A routine prescription from your home country is not enough at a Chinese border. Classification follows China's own catalogues of narcotic and psychotropic drugs, by active ingredient; how the drug is scheduled at home carries no weight. Methylphenidate, amphetamine salts, oxycodone, fentanyl patches, and benzodiazepines such as alprazolam or clonazepam are controlled. Medical cannabis, CBD and THC are banned outright. Compound painkillers are treated by their most restricted ingredient.

The legal logic

Smuggling narcotics is a crime regardless of quantity. Courts, though, are directed to treat carriage strictly for treatment and within reasonable limits as not criminal, and the State Council's narcotics regulations allow a person to carry controlled drugs for treatment, within the maximum of a single prescription, with a hospital diagnosis certificate and identity papers. Hiding the drug removes that protection.

How much is "reasonable"?

There is no fixed universal day-count. Customs officers weigh the maximum single prescription against reasonable personal use and your itinerary, referring to domestic prescribing limits. The practical benchmarks our source gives: narcotics about 15 to 30 days; Category I psychotropics up to about 30 days; Category II psychotropics about one to three months; injectables only for short emergency use. Never exceed 90 days. These are field practice, not statute, so confirm with the port before you go.

The dossier

  • Diagnosis certificate on hospital letterhead: name, date of birth, diagnosis, why this drug, daily dose, duration, physician licence, stamp.
  • Original prescription using the international generic name, with strength, form and total dispensed.
  • Authentication: an apostille for Convention states, or consular legalisation otherwise, plus a certified Chinese translation.
  • Original manufacturer packs with a label matching the passport name. No unmarked pill boxes.

At the port

Pack everything in carry-on. Pre-file through the customs "Fingertip Service" mini-programme or the portal, or fill in the paper form. Go to the red channel. Entering the green channel with controlled drugs is treated as concealment. Keep the stamped declaration slip for your whole stay and for departure.

Inside China

  • No mailed refills; parcels are seized.
  • Carriage is personal and non-transferable. Never carry for a companion or give any away.
  • Do not bin used fentanyl patches; fold, store and account for them.
  • Staying longer means registering at a Grade 3A hospital's international department with your records and declaration slip, where a doctor with narcotic prescribing rights can prescribe locally. Some drugs, notably mixed amphetamine salts, have no approved equivalent, so plan a transition with your doctor.
