Deductibles, co-pays and split hospital bills in China

Learn how GOPs, deductibles and co-pays affect Chinese hospital billing, which documents to collect, and what to confirm before a second insurance claim.

Insurance·

A guarantee of payment is a conditional billing arrangement, not a promise that the insurer will pay the entire hospital bill. The policy decides the deductible, co-pay, exclusions, limits and whether direct billing applies.

Read the bill as six numbers

  • the hospital’s total charge;
  • charges the insurer recognizes under the policy;
  • the remaining deductible;
  • the patient’s co-pay or co-insurance;
  • excluded or over-limit items;
  • the balance paid by the patient and the amount billed to the insurer.

Do not assume a common 10% or 20% rate. It belongs to the individual contract.

Three documents prove different things

The formal medical-charge receipt records actual medical revenue and payment information for public and other non-profit medical institutions. The itemized charge list explains what was charged. A hospital, insurer or TPA settlement statement explains how the policy allocated responsibility. National rules do not require every case to produce two separate receipts, and there is no single statutory “claims split sheet.” Electronic fiscal receipts can be valid documents; paper is not automatically superior.

Before discharge

  1. Get the written GOP amount, conditions and uncovered items.
  2. Compare the total ledger with the amount assigned to you.
  3. Save the formal receipt and complete itemization.
  4. Save the electronic receipt verification code or lookup details.
  5. Ask any second insurer what it needs from the first insurer and whether copies or electronic originals are accepted.

A second policy may still apply indemnity principles, so combined payment generally cannot exceed the actual covered loss. Sequence and documents depend on the policies. For a for-profit private hospital, ask exactly which fiscal document it issues rather than assuming the non-profit receipt rules apply.
